Case management is a consumer focused strategy for linking seriously mentally ill
(SMI) adults, seriously emotionally disturbed (SED) children, and individuals with mental retardation to necessary community support systems and services in order to improve their chances for recovery and/or successful community living, and to reduce the likelihood of consumers needing more intensive services. Most case management services and activities occur on an outreach basis. The primary goal of case management services is to ensure effective service coordination; i.e., assessment, linkage, support and resource development, advocacy, social support, case planning, service arrangement, reassessment and follow-up, and monitoring, to assist with meeting the consumer's human service needs.
